Community Health Worker- Full-Time $18-$21 an hour


Position Summary

A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of the community served (or has a very in-depth understanding of the community). This trusting relationship enables the CHW to serve as a link/between health and social services and the community. The CHW serves to facilitate access to services and improve the quality and cultural competence of service delivery.

The CHW will address social determinant of health (SDOH) needs through education, outreach, and partnerships with local organizations. The CHW will help individuals address the needs that lead to poor health outcomes, while assisting in accessing resources including quality health care, health insurance, health information, food, housing, and other social services.

The CHW will have a regular presence with the populations they serve and will provide support in accessing and using appropriate behavioral and physical health care.

While the Familylinks' CHW team covers multiple programs across Allegheny County, this opening will be primarily focused on the greater East End of Pittsburgh, PA working with the aging population and those with a diabetes diagnosis.

Assignments

The primary responsibility of the Community Health Worker is to:

Build trusting relationships with:

  • Community members, to connect them to needed services to support individual goals.
  • Partner organizations, to facilitate appropriate services.

Deliver Community education:

  • Develop and maintain relationships with community groups and leaders.
  • Deliver health education and campaigns on relevant health topics.
  • Build capacity within the community to address health issues.

Provide clinic visit support, as needed:

  • Attend select medical appointments with clients to assist in identifying needs and barriers to their care plan.
  • Support clients in meeting health and overall life goals.

Engage in collaboration:

  • Work with other CHWs to support clients and their care plans.
  • Work with supervisor and coworkers to support clients.
  • Work with partner organizations to develop master list of referral organization

Collect Information:

  • Participate in data collection and reporting activities required by Familylinks, funding agencies, and partner organizations.

Complete CHW training and obtain certification:

Full attendance in CHW training through Southwest PA Area Health Education Center or other accredited training organization is required; Familylinks will cover training and certification costs.

General Assignments

  • Record time and attendance via Human Resources Information Systems and Timekeeping system as applicable.
  • Attend meetings/training as requested/required.
  • Other duties as assigned.
  • Adhere to laws, regulations, and internal policies and procedures, including those related to maintaining client confidentiality such as HIPAA.
  • Participate in ongoing training and increase knowledge in the areas of community health work, aging services, social determinants of health, and healthcare.

Education & Qualifications

  • High School Diploma or equivalent required, some college course in social work, public administration, education, health or related field preferred.
  • Experience working with individuals fifty (50) years of age and older is preferred.
  • If using a personal car for travel, must provide valid driver's license.
  • Clean driving record, insurance and registration required.
  • Proficiency with Microsoft Outlook, computer/laptop, tablet, and cell phone required.
  • Proficiency with Microsoft Office Suite.

Working Conditions

  • Flexible scheduling is required. Work will be a mix of remote, in-home visits with program participants, office, and other community locations, all while maintaining HIPAA compliance.
  • Extensive local travel is required. Mileage reimbursement is provided.
  • Must provide own transportation.
  • Lifting up to 10 lbs. maximum and occasionally carrying such articles as files, ledgers and small tools.
  • A large amount of walking and standing may be necessary in carrying out duties.
  • This position is grant-funded, and therefore, it is limited in time to the duration of the grant. Additionally, target areas and populations could be adjusted over time.
